Forms During The Field Training 15 Weeks

Field Training Weekly log Sheet

Filled by trainee at the end of each week, send it by email to the academic supervisor, and at the end of each month submit by hand to the Academic Supervisor.

Filled by the trainee usually at the beginning of the 12^th week.

Filled by the field-training provider and submit with FTR in the presentation day.
